Your baby's birth must be registered within forty-two days (six

Where can I register my baby's
birth?
The birth of your child will be registered in the district in which
the child was born. You can give the information about your baby's
birth at any Register Office in England and Wales.

My baby was born outside the Borough
of Telford & Wrekin. Can I come along to Telford & Wrekin
Register Office?
Yes, we will take a Declaration of Particulars of your child's
birth to forward to the Register Office where the birth will be
registered. That office will post to you the baby's birth
certificate and form for the doctor. Please remember that these
will take a few days to arrive by post.
My baby was born at the Wrekin Maternity
Unit in the Princess Royal Hospital / at home in the district
of Telford & Wrekin.
We will register the birth for you and you will be able to take
away the child's birth certificate and form for the doctor at the
time of registration.
You are able to register a birth in the District of Telford &
Wrekin at our Wellington address on Monday to Fridays (see contacts
section).
Please note that from Monday 4th February 2008 you will need to
phone 01952 382444 for an appointment to complete any form of
registration.
On Wednesday mornings, by appointment only, registration is also
possible at the Library in Newport and Wednesday afternoons, by
appointment only, at Civic Offices Telford Town Centre.

Who can register the birth?
If the mother and father of the child were married to each other at
the time of the birth either parent may register.
If the mother and father of the child
were not married to each other at the time of the birth, the mother
alone may register the birth, but the father's details can only be
entered in the register, if he is present at the time of
registration or under certain circumstances (the Register Office
can supply you with more information about this). If there are
difficulties in attending to register or make a declaration of
particulars please telephone the register office to discuss the

What information will the
Registrar ask me for?
The Registrar will need to know:
- the date and place or birth
- If the baby is a twin/triplet
etc.
- the time of birth
- the sex of the baby
- the forename(s) and surname by which
the baby will be known and brought up
Details of mother:
- full forename(s) and surname
including maiden name if the mother is or ever has been
married
- date and place of birth
- current or last occupation
- if applicable, the date of
marriage
- if applicable, the number of children
she has by her present and any former husband
Details of father:
- If the father's details are to be
entered in the register we will need to know:
- full forename(s) and surname
- date and place of birth
- current or last occupation
- if applicable, the date of
marriage
Please note :
If the parents are not married to each other, the mother is not
obliged to have the father's details included in the entry. It may
be possible for them to be added at a later

Checking the
information.
The particulars that you give to the Registrar should be checked
very carefully before you sign the Register or the Declaration of
Particulars. These particulars are creating a very important
document for your child. Mistakes are troublesome to correct once
they are recorded in the Register.
Certificates.
After the birth has been registered, you will be given a short
birth certificate showing the baby's own details, free of charge.
This will meet most ordinary needs but a full certificate (a
complete copy of the entry in the register, showing parents details
and baby's details) can be purchased if required.
Full birth certificates and further short birth certificates can be
purchased at the time of registration or any time thereafter.
Certificates purchased at a later date will probably cost more than

How long will it take to register
my baby?
On average it takes between fifteen and twenty minutes to register
a birth. At the Register Office in Wellington you will need to make
an appointment by phone on 01952 382444 to register or make a
Declaration of Particulars.
