able to contribute to the good of their

These are the five outcomes of Every Child Matters
(government guidance). Agencies across the borough are working
together, using new shared practices, to improve these outcomes for
children, young people and their families. Telford & Wrekin has
received two best practice Beacon Awards for its work in this
area.
New Shared Practices
The Common Assessment Framework (or CAF) is a new way of
working with children and young people. It involves listening to
you and your child to find out your child's needs, and what is
working well in your child's life.
Agencies will get together with you and your child at a
Team Around the Child (or TAC) meeting, to agree a
plan of action with you, to make sure your child gets the right
sort of help.
The CAF is voluntary - you and your child can choose to be
involved.
The CAF will ensure that everyone involved with your child - such
as teachers and health visitors - work together to support your
child.
The CAF will help your child receive the right support at an early
stage before their needs increase. As the CAF is a shared
assessment, you and your child will not have to repeat the same
story to lots of different workers and you will be supported by a
lead professional throughout the process.

There is a dedicated Integrated Service Manager
(ISM) for your area who is supporting this new way of
working. They are responsible for managing a multi-agency team of
professionals who have lots of different skills and who may be able
