The reporting of Injuries, Disease and Dangerous Occurrences
Regulations 1995 places a legal duty on employers, the self
employed and those in control of premises to report some work
related accidents, disease and dangerous occurrences to the
relevant enforcing authority for their work activity. This can be
the Health and Safety Executive (HSE) or the Local Authority
(LAs).
When do I need to make a report? Death or major
injury: If there is a death or major injury to an
employee, or a self-employed person working on your premises or
- If a death or hospitalisation of a member of public
occurs.
You must notify the enforcing authority without
delay (e.g. by telephone) and then follow up with a completed form
(F2508) within 10 days.
Over three day injury: If there is an accident connected
with work and your employee, or self employed person working on
your premises suffers an over three day injury you must send a
completed accident report (F2508) form within 10 days.
Disease: If any employee is suffering from a disease
that is related to his work activity you must send a completed
accident report (F2508A) form within 10 days.
Dangerous occurrence: If something happens which
does not result in a reportable injury, but clearly could have
done, you must notify the enforcing authority
without delay (e.g. by telephone) and then follow up with a
completed (F2508) form within 10 days.

Who do I report to ? The Incident Contact Centre
(ICC), based in Caerphilly allows reports to be made to one single
point. The ICC is a joint ventre between HSE and LA's, it is user
friendly, simple, fast and effective to use.
There is no need to send another form to the HSE/ LA, the ICC takes
care of that and will send your report to the correct enforcing
authority.
