Adults with learning disabilities

The Adults with Learning Disabilities business unit, as part of the adult and consumer care care portfolio, delivers a range of services to meet our statutory requirements under the community care and other related legislation.

Telford & Wrekin Primary Care Trust and Telford & Wrekin Council, in conjunction with the Learning Disability Partnership Board have developed a strategy for adults with learning disabilities, to set out the vision and direction for the next three years.

Our primary functions are:
  • To protect vulnerable adults who live in the local community, and to provide social work support to adults with learning disabilities and their carers within the Telford and Wrekin area.
  • To co-ordinate an assessment and care management service that identifies need and monitors care for individuals.
  • To develop a multi-disciplinary approach to care, and to review all care packages as required.
  • To promote and assist with service changes and new initiatives to meet identified needs.
  • To involve users, carers and other relevant people in all aspects of assessment and service delivery.
  • To support individuals and carers to live in their own community.
  • To monitor and support residential placements, including those outside the council's area.

Overview of Services
Joint Community Learning Disability Team

  • Provide an assessment and care management, and review service or adults with learning disabilities.
  • Provide specialist nursing assessment and support.
  • Once eligibility for service has been established, we will directly provide or purchase care and support services for adults with a learning disability and their carers.
  • Provide advice, information and some services for those whose needs fall below our eligibility threshold.

The team is multi-discipliniary and works in partnership with
colleagues from South Staffordshire & Shropshire Healthcare NHS Foundation Trust who provide psychology, psychiatry, occupational therapy, speech & language therapy and physiotherapy.

The team also link with other services within the Council:

Person Centred Planning & Transition

  • To develop Person Centred Planning for adults with learning disabilities and to use PCP's as a vehicle to support the transition from children's services to adulthood.

Mental Impairment Rehabilitation and Advisory Service (MIRAS)

  • Supports people with a learning disability who offend and as such are deemed to have a mental impairment (Mental Health Act 1983). Provides assessment and care management and specialist service provision, including accommodation, access to work, education and leisure.

Community Support Services

  • Provides support to adults with learning disabilities living in their own tenancies or under licence with a number of Registered Social Landlords.

Residential Services

  • Provides residential services at Carwood in Stirchley and Downing House in Wellington for adults with a learning disability.

Adult Placement (shared care)

  • Provides respite and long term placements for adults with learning disabilities with an adult placement carer.
