Once we have decided on your
level of need, looked at any risks and decided that we can provide
a service, we will discuss with you what help can be offered. We
will let you know the range of services that are available and
discuss the types of help that would make it easier for you to live
as independently as
possible. | |

People whose needs fall below
the threshold may be helped to access services such as community
meals, aids to daily living,
housing related support
services and low levels of domestic support, particularly if
such services are identified as necessary to maintain independence
and prevent or delay the need for more intensive
services.
We will consider whether more
intensive, short-term therapeutic and/or rehabilitative support
(known as Intermediate Care) might enable you to regain your
independence or reduce the level of risk. If you are of working
age, we will also look to maximise your learning and employment
prospects.
If your needs are intensive, we will also provide a high level
of ongoing support services to enable you to live at home. These
might include day services, homecare, adaptations to your home or
breaks for carers.
