The Community Care Assessment Process

Under the NHS and Community Care Act 1990 local councils must follow these processes when deciding who should receive a community care service. There are a number of stages, the main ones are;

We need to make sure that members of the public know what services are available, how to get them and what they can expect.

The directory is one of these pieces of information and it lists other information that is available. It is currently being reviewed and a new version will be avaiable in the Spring 2007.

Assessment and Fair Access to services
This means finding out through discussion what your needs are and deciding whether your needs fall above the eligibility threshold set by the council. This also decides whether or not you have a right to a service. Carers also have a right to an assessment of their needs.

Care Planning and Service Provison
This means agreeing with you how best to meet your needs, within the resources available, and then helping you get the services you need. This also applies if you are a carer.

We will regularly monitor the services provided and make sure that they are meeting your needs. If your needs change we may complete a reassessment. This also applies to carers.

Assessment and Fair Acccess to Services
Guidance from the Government under the title 'Fair Access to Care Services' means that we have to set out the framework we use to decide why some people are entitled to a service and others are not. We call these Eligibility Criteria, and full details about how they work are available on request from your social worker or from any of the Social Services offices. Our first step is to always assess your needs.

We assess needs by looking at how far you are able to live independently. If your needs are above a certain level, Social Services must by law provide a service to you.

The Fair Access Guidance sets out four levels of risk to a person's independence. The highest level is "critical", followed by "substantial", then "moderate" and finally "low".

We have set the threshold for eligible need at "substantial" and above.

Even if your needs are below that level, and you are not entitled by law to a service, we will still try to provide a service if we can. We will discuss with you whether certain other lower level assistance could be offered.
