Anyone receiving a service from Adult Social Services
will need to give information about themselves and their
circumstances. This information helps us to make sure you are given
the best possible advice, care or support.
We hold this information in both paper and electronic format.
The type of information we hold is
- your name, address and date of
birth
- who referred you for assistance and why
- what your needs are and
- what services are being provided
Under the Data Protection Act, you can apply to see the personal
information that we hold.
No one else is entitled to see the information about you unless you
give us permission for it to be given to them.
To apply to see your information you need to complete an
application form.
We then have 40 days from receiving your request to make the
information available to you.
