Prevention & Rehabilitation

The Prevention & Rehabilitation team supports people with disabilities to become independant at home and in the community
The team works with individuals to help them develop and maintain skills and strategies needed to function in their homes and community. They can also liaise with providers of services and facilities in local communities to help break down barriers that disadvantage disabled people.
