Information you need to know about the Putting People
First in Telford and Wrekin transformation programme.
The programme commenced on 1 July 2008
The way in which we provide social care services for adults is
being transformed. The Government in its
Putting People First document has set out some of the biggest
challenges and changes for social care since the introduction of
the 1990 NHS and Community Care
Act. | |

The emphasis is now on the individuals dignity, the right
to choice, control and power over the services they receive.
The focus is on providing services at an early point, to help
prevent people requiring high levels of care over a longer
term.
This aims to help people remain independent for as long as
possible. Our approach will change to enable people to have far
greater control over their assessment, and more choice as to what
services will be delivered to meet their needs. This includes
access to information and the resources to access the services that
suit them and their personal requirements.
A transformation programme has been identified from which the
changes to our current systems will be made. This programme
commenced on 1 July 2008 and is expected to continue through to
2011.
