| Reason for consultation | There has been input from parents and professionals in developing the pack and we aim to ensure that the information in the pack gives clear and useful information and advice in a format that parents going through the diagnosis process will find helpful. |

| How to get involved? | Complete the questionnaire (please follow the link) and return by Friday 7th June 2013 at the latest. All of the responses are sent directly to the Quality Improvement Team and will be treated in the strictest of confidence, none of the responses will be identified with you personally. The survey will be analysed independently by one of the members from the Quality Improvement Team and a copy of the survey results will be sent to you once they are available. |
