Service delivery area Adults: Reablement and Prevention ( Adult Care and Support Services)
Reason for consultation The purpose of the consultation is to present a business case for the need for a multi-agency structured review of the current transition arrangements.
The project objectives are:
• Putting young people at the centre of the transition process
• Deliver on saving targets
• Agree Cross Council transition process
• Defined policies and procedures in collaboration with children services
• Review the current assessment processes within the introduction of greater emphasis on enablement services
• To agree commissioning and procurement arrangements for young people aged 16 years to 25 years.

The aims of the project are:

• Transition arrangements are holistic, inclusive and person centred
• Work towards improving outcomes for young people
• Work towards young people staying healthy
• Help young people have the best possible quality of life and to retain maximum dignity and respect
• Support and enable young people to achieve their maximum potential
• Prepare young people for greater independence as valued members of the community able to participate as active and equal citizens both economically and socially
• Ensure that each young person has the opportunity and is supported to make informed choices about decisions that affect their lives, including employment, housing, leisure & recreation and relationships
• Recognise that parents, carers, family and friends have an important contribution to make to the transition process.
Overview To gather commentary and feedback to five specific questions which will assist to improve the transition pathway for the future based on the the following vision statement:
• We recognise that people want to be as independent as possible and feel safe
• Sometimes people need extra support and when they do we will:
~ Give them good information to help yourself
~ Help you regain skills you have lost and develop new ones
~ Help you have choice and control over the support you need.

Young adult who are carers’ and parent carers will be asked to comment on the following five questions:
1. Is the vision statement relevant to young people in transition, if not how would you change it?
2. Describe your perfect transition process?
3. What needs to change in Telford and Wrekin?
4. Can this be achieved by a single team process?
5. What would a transition plan include and what would it look like?
How to get involved? Planned events:
3 October 6pm – 7.15pm at The Place
13 October: 11am - 1pm at The Glebe.
Invitations have been sent out to known young adults and parent carers to contribute to the consultation.
What happens next? Information collected will be evaluated by the Project Initiation Group.
