Adult Social Care Charter

Our charter has been co-produced with staff and members of our Making It Real Board. These experts by experience have worked alongside us to shape the Adult Social Care Charter, ensuring it reflects the principles that matter most to them.

The charter is built around five overarching commitments that guide all staff when supporting individuals and the people who are important to them.

  • We apply a strengths-based ethos throughout our interventions with people.
  • We ensure holistic assessments are completed, with a multi-agency approach.
  • We ensure that all visits are meaningful to the person and their families/carers.
  • We capture our assessments as a narrative. The emphasis is about having meaningful and relevant conversations.
