Contact the Emergency Duty team for urgent situations, outside of normal hours, which cannot be left with an appropriate degree of safety until the next normal working day, including weekends and bank holidays.

If you care for a family member or friend without pay, you might be eligible for a carer’s assessment - a chance to explore support that could help you in your caring role.

A Care Act assessment helps identify the support you need to stay independent by looking at how you manage daily life, including personal care, work, and routine tasks.

You may need to contribute to care costs, based on government guidelines. We're happy to help with any questions about financial assessments, contributions, or invoices.

We will assess your needs and home to suggest equipment or techniques that support independence. We may loan equipment and consider home adaptations if needed.

If your health changes suddenly and you need hospital care, the Telford Integrated Community Assessment Team (TICAT) become involved to help with your discharge out of hospital.

A direct payment is one way to manage your personal care budget, you will receive cash payments from the Council, following an assessment by adult social care, to arrange and pay for the care and support services you need.
