Overview of activity:

 

We want to make sure people understand what is happening when they work with Adult Social Care. This includes before, during and after assessments and when they make decisions about their care and support.

How we plan to do this:

 

We are planning to create clear and helpful information so that residents and professionals know:

  • What Adult Social Care does
  • What Adult Social Care does not do
  • What to expect at each stage of the process

This will include:

 

  • A simple guide through the service
    This will show people where they are in the process, what happens next, and who they can contact if they have questions or worries.
  • An easy to use jargon buster
    This will explain words and phrases that can be confusing, linked to parts of the journey through Adult Social Care such as:
    • Care Act assessments
    • Financial assessments
    • Carers assessments
    • Direct payments
  • Clear and accessible information
    With your help we would like to check all our information to make sure it is easy to understand. We will also make sure people know how to ask for information in different formats, such as Easy Read, large print, or other formats they may need.

Skills and experience required

 

No experience or knowledge is needed as this information is not only for people who currently access Adult Social Care and professionals but also members of the public who do not know anything about Adult Social Care.

Who can get involved

 

Adults over the age of 18 who may need support from Adult Social Care or who are already in receipt of Adult Social Care services including those who are currently transitioning from Children’s services into Adult Social Care.

Support available from the co-production team, please advise on your expression of interest what support would help you to take part.

Expected start/end date

 

Hybrid meetings (attend in person & via Microsoft Teams) will be held on a regular basis once a start date is identified.
